JOB RESPONSIBILITIES:

  • Assistant new hires through appropriate orientation, evaluation, assignment, and delegation of duties.

  • Assure staff, resident, and visitor compliance with policies and procedures.

  • Ensure residents, visitors, and staff have a safe, clean, and comfortable environment.

  • Know the emergency preparedness plan and be able to react and give instructions as indicated.

  • Attend in-service programs and staff meetings.

  • Personally responsible for shift coverage when unable to cover any other way.

  • Use good judgment in notification when there is a change in the resident’s physical or mental condition.

Assisted Living Caregiver



JOB RESPONSIBILITIES:

  • Follow established performance standards and implement facility policies and procedures on the unit assigned.

  • Interpret existing policies and procedures to CNAs, residents, families and physicians.

  • Assume responsibility for compliance with federal, state and local regulations within the unit on the assigned shift of duty.

  • Instruct CNAs in proper, preventive measures and use of adaptive equipment to meet the needs of each resident.

  • Provide job performance counsel and mentoring to supervised CNAs – recommending, when necessary and appropriate, disciplinary action to  D.O.N.

  • Provide clinical supervision and daily instruction to CNAs regarding care relating to ADLs.

  • Supervise and evaluate all direct resident care provided and initiate corrective action as necessary.

  • Obtain report from nurse being relieved and record sufficient information to implement appropriate follow-up action as necessary in relation to the resident’s treatment needs.

  • Conduct resident rounds.

  • Report resident health care changes to DON- ADON-MDS (as appropriate), including development of pressure sores, and take follow up action.

  • Obtain physicians’ orders and any revisions for all treatments needed.

  • Start IVs and draw blood when necessary, if RN, and appropriate related functions, if LPN.

  • Document resident care provided and resident’s response, or lack of response, to treatment provided.

  • Assess resident needs and participate in development and maintenance of individualized resident care plans in compliance with care plans.

  • Participate in resident care conferences as requested.

  • Provide direct resident care.

  • Provide concise, accurate report to staff nurse coming on duty and take follow up action as necessary.

  • Supervise and evaluate the implementation of “Residents’ Bill of Rights”.

  • Identify safety hazards and initiate corrective action.

  • Identify special nursing problems and emergency situations quickly and initiate immediate “life saving” measures in the absence of a physician, unless Advance Directives indicate otherwise.

  • Responsible for the safety of residents under his/her supervision.

  • Observe and assure infection control procedures.

  • Listen to resident, family, or physician complaints and, as appropriate, initiate corrective action and/or report special problems to the DON-ADON.

  • Control supplies and equipment.

  • Compile information for facility committees as instructed.

  • Assist in/complete the admission, transfer and discharge of residents as necessary/assigned with appropriate documentation.

  • Attend and participate in continuing educational/inservice/professional staff programs as required.

RN/LPN

JOB RESPONSIBILITIES:

  1. Supervises the Rehab Aide, students, and volunteers as assigned.

  2. Provides therapeutic interventions in accordance to the treatment plan, assists to return to as high of a functional level of activity as possible, under the supervision of an Occupational Therapist.

  3. Documents daily clinical interventions and occurrences which includes skilled observations, objective base lining weekly, and response to treatment.

  4. Maintains appropriate resident schedule to assure compliance with treatment plan and PPS guidelines.

  5. Records daily billing documentation.

  6. Adheres to the American Occupational Therapy Association Standards of Practice.

  7. Participates in care conferences and required in-services as indicated.

  8. Participates in the departments QI as assigned.

  9. Participate in activities to assure and facilitate effective department operations.

  10. It is part of my job duties to ensure resident safety.

COTA

JOB RESPONSIBILITIES:

  1. Build relationships in the community by actively participating in events sponsored by community and civic organizations.

  2. Develop an annual marketing plan which supports the short and long term vision for Thurston Woods Village.

  3. Monitor and report on key performance indicators related to marketing efforts, such as census, wait list and lead generation.

  4. Monitor and report on performance indicators related to website traffic, social media and online engagement.

  5. Work with Directors of our communities to monitor follow through on leads for routine reporting on sales activity.

  6. Complete a competitive analysis at least annually, using a consistent assessment tool.

  7. Responsible for digital presence including social media activity, regular updates, promotion of events and services, generate positive reviews, monitoring website for updates and email campaigns.

  8. Develop and implement a plan to maintain regular contact with the community, sponsors and donors in the form of regular newsletters and an annual report.

  9. Educate the community on new projects and incorporate a marketing strategy into the overall plan.

  10. Develop materials (flyers, brochures, etc.) to be used to promote events and services.

  11. Responsible for creation of media, such as print ads, radio spots and other outlets.

  12. Oversee the planning and implementation of fund raising events, gathering a team of staff and volunteers to assist.

  13. Prepare a marketing and fund raising budget of projected revenue and expenses and maintain compliance with the budget.

  14. Order equipment and supplies for the department as needed.

  15. Serve on various committees within the facility and outside the facility as may be appointed by the CEO.

  16. Attend workshops/seminars as necessary to keep abreast of new approaches and developments regarding philanthropic activities.

Marketing Director

JOB RESPONSIBILITIES:

  1. Evaluate the patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self–care ability, work capacity, etc.) and participate in the development of the individualized plan of care.

  2. Develop a treatment program and establish goals for improved function. Communicates plan of care to referring physician and other organization professionals. Also necessary is to develop a program for caregivers' energy conservation.

  3. May teach new skills or retrain patients in using various types of adaptive equipment to improve function.

  4. May fabricate splints and instruct patient in familiar daily activities lost due to illness or injury.

  5. May train patient and family/caregiver in the use of prosthetic and/or orthotic devices.

  6. Maintains appropriate clinical records, progress notes, and reports to the physician any changes in the patient's condition.

  7. Maintains contact/communication with team members in the patient’s care to promote coordinated, efficient care.

  8. Attends and contributes to in-services, interdisciplinary case conferences, and other meetings as required by organization policy to comprehensive assessments for the patients or the organizations.

  9. Instructs and educates patient’s/family/caregiver and other team members in patient’s treatment regime.

  10. Actively participates in quality assessment performance improvement teams and activities.

  11. Participates in discharge planning for patients.

Occupational Therapist
