Short Term Rehabilitation

Residents admitted to Froh Community Rehabilitation are expected to be discharged home or to a facility with less care, as quickly as possible. Providing up to seven days a week of intense therapy, where indicated, is our norm. Short term rehab residents are provided upwards of three hours of therapy daily in our short term therapy department where they relax in the living/TV/fireplace area, eat meals together in the dining room located in the unit, prepare meals and enjoy other interests when not directly involved with direct 1:1 therapy.

Our in house Occupational Therapists, Physical Therapists and contractual Speech Therapists work closely with the resident and families; meeting within the first 5 days of being admitted to the unit to discuss the plan of care and treatment program. Home evaluations are a standard along with the discharge care plan meetings to assure that we have met all the goals so that the resident may return home as safe and quickly as possible.

Planning for home care therapy is important aspect for total rehabilitation and we refer to a resident‘s choice for those services may it be Nursing, Skilled Therapy, Home Health Aides, Meals on Wheels and other services to assure a safe lasting discharge.

We are proud of our short term rehabilitation outcomes, sending those residents home as quickly as possible. We excel in knee and hip rehabilitation with our length of stay for knee replacements averaging 7 days, and depending on the surgical procedure, 14 to 30 days for hip repair. Often a resident is admitted to rehabilitation after a debilitating illness to just regain strength and function and return home quickly.

Froh Community Home and Rehabilitation Center is recognized by the Centers of Medicare and Medicaid (CMS) as a 5 Star Facility-a distinction that is given to less than 5% of nursing facilities nationwide.

Long Term Rehabilitation

Recognizing that many residents require continued nursing and general medical care, the rehabilitation department provides skilled therapy services to assure a resident performs at their maximum functional potential. Often times a resident will show a decline in function such as walking, strengthening, fine motor skills, dressing, etc… and needs skilled therapy services to return them to their prior level of function. Froh Community Home is one of a very few skilled care facilities that offer restorative therapy services with a fully staffed restorative department seven days a week to provide the best possible care for your loved one.

“The resident satisfaction surveys continue to indicate we provide quality of care that is expected, with the survey results showing 98% excellent satisfaction rating,” states Theo Omo, CEO. Theo continues to say, “This is one of the reasons Froh Community Home and Rehabilitation Center is recognized by the Centers of Medicare and Medicaid (CMS) as a 5 Star Facility-a distinction that is given to less than 5% of nursing facilities nationwide.”
